Deposit for Change of Zone Application (REZ 10-000126) <br />.Applicant — Kona Country Club, Inc. <br />Tax Map Key 7-8-010:101 <br />This is to acknowledge receipt of the required $28,800.00 water commitment deposit for a commitment time <br />extension for the proposed development. We are enclosing Receipt No. 295552 for your files. <br />Pursuant to Rule 5 of the Department's Rules and Regulations, a water commitment for the proposed <br />development in the amount of 19,200 gallons per day, or forty-eight (48) additional units of water at an average <br />of 400 gallons per day, per unit, is hereby granted until July 31, 2020, with the following conditions: <br />I . A Water Master Plan, prepared by a professional engineer licensed in the State of Hawaii, showing <br />how the applicant proposes to provide water at adequate pressure and volume under peak -flow and <br />fire -flow conditions. Extensive improvements may be required, which may include, but not be limited <br />to, additional booster pump, transmissions, and storage facilities. <br />2. Construct necessary water system improvements, which shall include, but not be limited to: <br />a. water mains capable of delivering water at adequate pressure and volume under peak -flow and <br />fire -flow conditions, <br />b. installation of a service lateral that will accommodate the appropriate meter size to each lot, <br />C. subject to other agencies' requirements to construct improvements within the road right-of-way <br />fronting the property affected by the proposed development, the applicant shall be responsible <br />for the relocation and adjustment of the Department's affected water system facilities, should <br />they be necessary, and <br />d. fire hydrants spaced no more than 300 feet apart and within 150 feet of the driveway or access <br />for each lot. On dead-end streets, the last fire hydrant shall be located at one-half the distance <br />from the last house, or unit, fronting the property line, or to the driveway or access for the <br />property. <br />Submit construction plans and design calculations prepared by a professional engineer licensed in the <br />State of Hawaii, for review and approval. <br />Planning Deet. ...
