County of Hawaii <br />201H Application Package <br />Tenants with Special Housing Needs 101 + 155 = 256 � <br />(please describe the Special Housing Need of the tenants below) <br />As noted in footnote 1 above, Phase I of the Project shall consist of an Assisted Living/Memory Care Facility <br />serving residents with assisted living services including, without limitation, daily meal service, housekeeping <br />services, social activities, transportation and personalized assistance from trained caregivers and nursing staff. <br />The memory care component will specifically cater to residents with Alzheimer's disease, dementia and other <br />types of memory problems. Phase II shall consist of a Skilled Nursing Facility, which will provide 24 -assistance <br />with activities of daily living, skilled nursing and paramedical care, and programs for short-term rehab as well as <br />long-term care. <br />*Please refer to the definition of Elder or Elderly Households as defined in Act 64, Session Laws of <br />Hawaii 2002 <br />20%); and (iii) Phase III: Employee Housing with accommodations for employees of the AL/MCF and SNF if and to the extent <br />sufficient water capacity is available to the subject property. <br />Form 201H-001 Page 2 <br />
