EXHIBIT B-2 <br /> REQUISITION NO._ <br /> (Project Fund) <br /> HAWAII COUNTY <br /> COMMUNITY FACILITIES DISTRICT NO. 1-2021 <br /> (KALOKO HEIGHTS PROJECT) <br /> REQUISITION FOR DISBURSEMENT <br /> OF COSTS OF ISSUANCE OR IMPROVEMENT COSTS <br /> U.S. Bank Trust Company, National Association, as Trustee (the "Trustee"), is hereby <br /> requested, pursuant to the Trust Indenture dated as of , 2022 (the "Indenture") between the <br /> Trustee and County of Hawai`i (the "County"), to pay the amount(s) specified in Schedule I attached <br /> hereto, such payments to be made from the Account(s), to the payee(s) and for the purpose(s) <br /> (consisting of Costs of Issuance, Incidental Expenses and/or Actual Costs of the Improvements, as <br /> such terms are defined in the Indenture) specified in such Schedule I. Such payment(s) is(are) to be <br /> made by check or wire transfer in accordance with payment instructions set forth in Schedule I. <br /> The amount(s) requisitioned hereunder is(are)due and payable under purchase order, contract <br /> or other authorization and has not formed the basis of any prior request for payment.
