Program Objectives & Performance The next set of questions will provide grant application reviewers a clear sense of what your proposal is designed to accomplish and what is needed to get there. The examples below relate to a fictional feeding program. Please keep in mind that character counts include punctuation and spaces. 1.In 500 characters,what is the community need the program intends to fulfill?Along with the narrative,please include data and/or citations documenting/measuring the need.* Mental Health America's 2022 study,ranks Hawaii as the state with the largest percentage of adults(67.1%)with mental illness as not being treated.Also,there is a 33%shortage of psychiatrists on the Big Island.Economic security is another need as Hilo's poverty level(20.06%)is higher than the national average of 12.8%.This problem is worse for our members;90%of them live below the poverty line.The Clubhouse serves these individuals coping with lack of mental health care and poverty. 2.In 500 characters,what is the program goal?Describe how your program advances the CHANGE category you picked and creates a Ripple Effect of positive impact.What is the clear desired future state that correlates to the end of the one-year program period?* Our CHANGE category is Health/Wellness.We address this by providing mental health(MH)services and linkages to case management(CM) ,often filing a gap when consumers are without MH CM and/or need MH crisis support,reducing demands on MH providers and ER staff,a huge cost saving to the community.Addressing poverty,we offer discounted meals,free holiday meals,classes in nutrition,cooking& shopping.Training in 1 year will significantly improve MH services and reduce food insecurity. 3.In 1,000 characters,list what resources are needed to carry out your program,effort,or initiative.What do you need in order to carry out the activities at the frequency and scale needed to achieve the outputs?These are your INPUTS.Hint:this should look similar to the narrative section of the detailed budget request and include all types of in-kind services and contributions.* We request funds for training($40,000).Our plan is to send 2 members/2 staff to mainland Clubhouse International generalist trainings ($28,000).send our director to mainland director training($5,700)and send 1 staff/1 member to Oahu for on-site training in Clubhouse operations($6,300).Our request is larger than last year because Clubhouse International is no longer offering virtual training,forcing us to spend more money on travel.These trainings will provide important education,guidance/support in the Clubhouse Model and assist us in obtaining/maintaining Clubhouse International Accreditation,required by the State of Hawaii.Currently,only 5 staff and 5 members out of 86 members have been trained in generalist training and our director did not received director training as planned for in 2023.In spring 2023,we are sending 2 staff/2 members to generalist training.By the start of fiscal year 23-24 we will have 7 members/5 staff trained in in-person generalist training. 4.In 1,000 characters,list the MAJOR ACTIVITIES to be completed.To achieve the indicators you listed,what would you need to do?These action statements begin with a verb and normally includes a timeline.For example:design and collect materials for STEM kits by month 4.* Major Activities planned:1)Sending 2 staff/2 members to Clubhouse International training in Spring of 2024.2.)Sending director for training in Spring 2024.3.)Sending 1 staff/2 members to Oahu for Clubhouse operational training in Spring 2024.4.)Staff and members will create an action plan at each of the training sessions.5.)Staff and members will bring back the action plans and work within Clubhouse to put them into practice within 1 month of training completion.6.)Applying for 3 year Clubhouse International accreditation by June 30th,2024.7.)Reporting to Clubhouse community and community at large(Board members,donors,stake holders)about progress by June 30th 2024 8.)By June, 2024,increase yearly outreaches to 6 fundraisers/7 presentations to educate the community at large and also Hawaii lawmakers about individuals with serious and persistent mental illness;their courage in the face of stigma and their unique needs and potential contributions to our community. 5.In 1,000 characters,list the outputs to be completed.What are the quantifiable results of those activities that you would need to obtain in order to achieve the indicators you listed?These are OUTPUTS.Each output begins with a number and directly correlates to a specific activity. One director will attend director training.3.)1 staff and 2 members will be sent to Oahu for Clubhouse operational training.4.)3 action plans will be created for each training.5.)3-year Clubhouse International accreditation will be awarded.6.)One special newsletter will be published and sent out to all constituents,funders,donors,employers and board members to keep the public abreast of progress.7.)6 community fundraising events and 7 community Zoom outreach presentations will be conducted.8.)Current membership base of 86 will receive Clubhouse International training-informed services and membership will increase by 5%.9.)10%increase in active member's and staff's understanding of the Clubhouse Action Plan.
