include data and/or citations <br /> documenting/measuring the need.* <br /> Mental Health America's 2022 study,ranks Hawaii as the state with the largest percentage of adults(67.1%)with mental illness as not being <br /> treated.Also,there is a 33%shortage of psychiatrists on the Big Island.Economic security is another need as Hilo's poverty level(20.06%)is <br /> higher than the national average of 12.8%.This problem is worse for our members;90%of them live below the poverty line.The Clubhouse <br /> serves these individuals coping with lack of mental health care and poverty. <br /> 2.In 500 characters,what is the program goal?Describe how your program advances the CHANGE category you picked and creates a Ripple <br /> Effect of positive impact.What is the clear desired future state that correlates to the end of the one-year program period?* <br /> Our CHANGE category is Health/Wellness.We address this by providing mental health(MH)services and linkages to case management(CM) <br /> ,often filing a gap when consumers are without MH CM and/or need MH crisis support,reducing demands on MH providers and ER staff,a <br /> huge cost saving to the community.Addressing poverty,we offer discounted meals,free holiday meals,classes in nutrition,cooking& <br /> shopping.Training in 1 year will significantly improve MH services and reduce food insecurity. <br /> 3.In 1,000 characters,list what resources are needed to carry out your program,effort,or initiative.What do you need in order to carry out the <br /> activities at the frequency and scale needed to achieve the outputs?These are your INPUTS.Hint:this should look similar to the narrative <br /> section of the detailed budget request and include all types of in-kind services and contributions.* <br /> We request funds for training($40,000).Our plan is to send 2 members/2 staff to mainland Clubhouse International generalist trainings <br /> ($28,000).send our director to mainland director training($5,700)and send 1 staff/1 member to Oahu for on-site training in Clubhouse <br /> operations($6,300).Our request is larger than last year because Clubhouse International is no longer offering virtual training,forcing us to <br /> spend more money on travel.These trainings will provide important education,guidance/support in the Clubhouse Model and assist us in <br /> obtaining/maintaining Clubhouse International Accreditation,required by the State of Hawaii.Currently,only 5 staff and 5 members out of 86 <br /> members have been trained in generalist training and our director did not received director training as planned for in 2023.In spring 2023,we <br /> are sending 2 staff/2 members to generalist training.By the start of fiscal year 23-24 we will have 7 community Zoom outreach presentations will be conducted.8.)Current membership base of 86 will receive <br /> Clubhouse International training-informed services and membership will increase by
