On June 5, 2017, a pipe failure caused secondary treated and disinfected wastewater <br /> to discharge to coastal waters near the Pua sewage pump station. <br /> h. On April 3, 2017, approximately 41,000 gallons of sewage spilled from the manhole at <br /> Waianuenue and Laimana Street intersection in Hilo, reaching Wailuku River and the <br /> Pacific Ocean. The spill was due to debris blockage in the gravity sewer line. <br /> i. On January 27, 2017, approximately 5,000 gallons of sewage spilled from the Paukaa <br /> pump station along Paukaa Drive in Hilo, reaching the Pacific Ocean.The spill was due <br /> to rainfall overwhelming the capacity of the pump station which led to the wet well <br /> overflowing. <br /> 25. Based on review of the records provided by Respondent and observations during <br /> inspections, Respondent does not have a program in place to systematically repair, rehabilitate, <br /> and replace aging infrastructure prior to failure. <br /> 26. Based on a review of the records provided by Respondent and observations during <br /> inspections, Respondent does not have a comprehensive capital improvement planning (CIP) <br /> program to evaluate criticality of infrastructure and to plan for anticipated failures.
