Homeless Task Force's Outreach and Engagement directly assists homeless individuals and families in District 8,induding the Honokohau Harbor area.Through this initiative,the Homeless Task Force conducts outreach to aGc the immediate needs of the unhoused—providing food cards,water,hygiene kits,dothing cards,and transportation.The project connects individuals to vital services such as mental health counseling,medical clinics,housing programs,substance abuse treatment,job training,and family reunification support.Outreach workers offer case management and develop client-centered plans that reflect each individual's preferences and goals.All support and expenditures are documented in client files and tracked in daily spreadsheets.This holistic,individualized approach aims to build trust and create pathways toward stability,with the ultimate goal of helping clients transition off the sheets and into safe,permanent housing or other supportive solutions. *42.501(c)(3)Mission Statement Our mission is to decrease the homeless count in West Hawaii by providing individuals with a productive pathway back into mainstream society through safe haven options of housing,drug and alcohol rehabilitation referrals,and return home to their families of origin. *43.In 1,000 characters or less,explain how the 501(c)(3)is connected to the project. With the requested funds,the Homeless Task Force aims to Interview 40 individuals and provide direct case management to 30 individuals in District 8.This viable effort will encourage the unhoused in the area to move from the homeless lifestyle and transition into safe haven options,thereby decreasing the homeless count in the area and changing the trajectory of their lives for the better.
