In 2,000 characters or less, describe the project. <br />The North Kohala Farm to Family Project is a targeted food access initiative designed to address persistent food Insecurity and diet -related chronic health conditions <br />among low-income residents in our rural North Kohala community. Through this program, approximately 200 individuals from 50 qualifying households —those living at or <br />below the poverty line and/or managing chronic health issues —will receive free, locally grown produce bags at a value of $27.50 each on a bi-weekly basis. This 4- <br />month program, proposed for September through December 2025, not only improves access to fresh, nutritious food but also strengthens our local agricultural economy <br />by supporting area farmers and building resilience within our regional food system. Participant testimonials from our successful Veggie Rx and previous Farm to Family <br />programs underscore the positive health and quality -of -life impacts of regular access to fresh produce. With continued support, the Farm to Family Project will fill a <br />critical gap for vulnerable families and Individuals in North Kohala, creating a healthier, more food -secure future for all. <br />*47.501(c)(3) Mission Statement <br />Increasing local food Infrastructure, coordination and access for a resilient and abundant Kohala. <br />*48. In 1,000 characters or less, explain how the 501(c)(3) is connected to the project <br />Feed Hawaii DBA Kohala Food Hub, a 501(c)(3) nonprofit, is the lead organizer of the North Kohala Farm to Family Project, building on our successful coordination of the <br />2025'Umeke'Ai Pono program, 2024 Farm to Family program, and 2022-2024 Veggie Rx program. We will source produce from over 15 local fanners and growers, <br />manage purchasing, packing, and weekly distribution, and serve as a central pick-up site. Our beam will also handle participant outreach, enrollment, communications, <br />order tracking, farmer payments and budget management. Our proven experience over multiple years managing similar programs demonstrates our strong capacity to <br />deliver the 2025 Farm to Family Project effectively. <br />*49. In 2,000 characters or less, describe the measurable outcomes you aim to achieve with these funds. <br />The North Kohala Farm to Family Project will deliver measurable outcomes that directly address food insecurity, improve community health, and support the local <br />agricultural economy. Over the 4-month program period (September -December 2025), we aim to achieve the following: Food Access: Distribute 400 produce bags <br />(valued at $27.50 each) to approximately 200 individuals from 50 qualifying households, providing consistent access to fresh, nutrient -rich fruits and vegetables. Health <br />Improvement: At least 75% of participants will report increased fruit and vegetable consumption and a greater awareness of healthy eating habits, as measured by pre - <br />and post -program surveys. Community Impact: Reduce food insecurity for all participating households, with 80% reporting improved healthy local produce availability. <br />Economic Support: Direct $7,600 in produce purchases to local farmers and backyard growers, reinforcing our regional food system and generating income for small- <br />scale producers. Program Efficiency: Maintain a high level of satisfaction with program delivery, with 90% of participants rating communications, produce quality, and <br />pickup logistics as "good" or "excellent." Data collection will Include enrollment tracking, produce bag distribution logs, expenditure records, and participant feedback <br />through surveys and testimonials. These outcomes will guide program improvement and demonstrate the ongoing need for sustained support. <br />*52.
