In 2,000 characters or less, describe the project <br />Purchase of school supplies for our Kohala schools Including: Kohala elementary, HaWula Middle School and Kohala High School. These tools for success will allow <br />families to be relieved of the financial burden of purchasing school supplies for their children. We have found that about 62% of the North Hawari population are <br />qualified for free or reduced lunch hence the need for assistance in this area. <br />*46.501(c)(3) Mission Statement <br />Provide quality healthcare that is responsive to our patients' and communities' needs. <br />*47. In 1,000 characters or less, explain how the 501(c)(3) is connected to the project <br />The 501(c)(3) organization provides quality healthcare responsive to community needs, which includes addressing social determinants of health like education. By <br />supporting school supply access for Kohala families-62% of whom qualify for free or reduced lunch —the organization helps reduce financial stress, promote academic <br />readiness, and support overall well-being of the North Hawari community It serves. <br />*48. According to §2-136, "public purpose" is defined as a benefit to, Improvement of, or promotion of the interests in the following areas. Please select all the public benefits <br />to be derived from your County fund request: <br />Needs of the poor, youth, the aged, those with physical or emotional disabilities <br />*50. In 2,000 characters or less, describe the measurable outcomes you aim to achieve with these funds. <br />With this funding, we aim to distribute school supplies —including backpacks and wide -ruled composition notebooks —to students at Kohala Elementary, Hala'ula Middle, <br />and Kohala High School before the school year begins. We expect to reach at least hundreds students across the three schools, prioritizing those whose families qualify <br />for free or reduced lunch. This project supports educational equity, student success, and family well-being, which all contribute to long-term health outcomes in our <br />North Hawari communities. <br />*51.
