Exhibit J —Award Nominee Testimonies <br />Testimony from Lokahi Treatment Centers Recovery and Housing Stability Program <br />"in June 2025, an 18-year-old part -Hawaiian female was referred to Lokahi Treatment <br />Centers by Child Welfare Services (CWS). As a young mother who had recently aged out of <br />the foster care system, she faced significant challenges, including unstable housing, <br />Limited support, and the emotionaltoll of being separated from her one -year -old son. She <br />and the child's father (no longertogether) were both referred to outpatient treatment to <br />address substance use concerns and strengthen their readiness for reunification with their <br />child. <br />At admission on June 10, 2025, she was living temporarilywith a friend in Hawaiian Acres, <br />an arrangement that provided short-term shelter, but lacked the stability needed to meet <br />CWS reunification requirements. Recognizing the urgent need for a safe and stable living <br />environment, our team immediately connected her with resources tailored to young adults <br />transitioning out of foster care. She was referred to The Salvation Army's Transitional <br />Housing Support, which specifically assists youth who age out of foster care and often face <br />homelessness. <br />Through her active participation in treatment, consistency in attending sessions, and <br />willingness to utilize community resources, she was able to secure housing in late October. <br />To help her create a safe and functional home for her and her child, she was also <br />connected to United Way, which provided essential furniture and household items. These <br />supports not only strengthened her recovery foundation, but also accelerated the timeline <br />for reunification with her son. <br />Throughout her time in Lokahi Treatment Centers Outpatient Program, she demonstrated <br />determination, resilience, and a growing confidence in her ability to rebuild her life. With <br />staff approval, she successfully completed her treatment program. Although formally <br />discharged, she remains engaged with Lokahi Treatment Centers Monitoring Program", <br />allowing us to continue supporting her progress and ensuring ongoing stability in her <br />recovery and housingjourney. <br />Today, she is well on her way to creating a safe, healthy, and nurturing home for her child, <br />an achievement made possible through her own commitment, the dedication of the Lokahi <br />Treatment Centers team, and the supportive partnerships within our community and <br />through this Homeless and Housing Fund Grant. <br />Her story reflects the purpose of our integrated Substance Abuse, Mental Health, and <br />Recovery & Housing Stability services: to help individuals heal, rebuild, and thrive, one step <br />at a time:' <br />Res. 397-25 Homelessness and Housing Fund Yr 4 December 17, 2025 <br />
