Please select all the public benefits to be derived from your County fund <br /> request: <br /> Culture and the arts <br /> Educational concerns <br /> Needs of the poor,youth,the aged, those with physical or emotional disabilities <br /> Public health and welfare of the people and the environment • <br /> Victims of crimes <br /> Victims of health or social crises <br /> * <br /> 47. In 2,000 characters or less, describe the measurable outcomes you aim to achieve with these <br /> funds. <br /> The Summer Jam project aims to bring together approximately 700 attendees for a safe, positive, and substance- <br /> free community event that promotes wellness,youth engagement,and community connection. A key measurable <br /> outcome of the project is the participation of at least 30 health fair and community resource vendors that will <br /> provide information, education, and supportive services related to health, prevention,wellness, behavioral health, <br /> and community resources.These funds will support event coordination, outreach, and activities that help ensure <br /> strong community participation and successful engagement between attendees and local organizations.The project <br /> will measure success through total attendance numbers and the number of participating vendors and community <br /> partners involved in the health fair portion of the event. <br />
