(AHPI). The <br /> purpose of the conference was to discuss the February 12, 1997, operation of <br /> a megacurie pool irradiator in Puerto Rico for a period of 2.5 hours without <br /> a licensed irradiator operator present at the facility. The license had been <br /> held by AHPI at the Time of the event, but was transferred to Isomedix on <br /> March 19, 1997. The licensee discussed the circumstances surrounding the <br /> <br /> event, causes, and corrective actions. <br /> <br />
