The project is <br /> funded by the U.S. Department of Justice, Community Oriented Policing (COPS) federal grant program (the <br /> "Federal Grant") and is subject to the conditions of that program. We are excited about your project and believe <br /> it is the kind of work that can help to make a difference in Hawaii. <br /> Proposal ID#: 20041397 Grant Award: $200,000.00 <br /> Grant Period: 9/1/2004 to 12/31/2005 <br /> Grant Purpose: Drug lab equipment <br /> Final Report: Due 30 days after the grant end date <br /> <br /> Please read the followine Brant conditions carefully: <br /> • This grant is to be used only for the purposes set forth in the proposal and work plan you submitted to <br /> HCF for the Hawai' i Island Meth Initiative as mutually amended from time to time (the "Proposal"). If <br /> there is any change in any of the activities described in the Proposal, please contact us before expending <br /> the funds. <br /> • As grantee, you acknowledge that the grant is being made in connection with the Federal Grant. You <br /> further acknowledge and agree that you are familiar with the requirements and conditions of the Federal <br /> Grant and agree to comply with all requirements that maybe applicable to your organization as a sub- <br /> recipient of the Federal Grant as further set forth in Exhibit "A", which is attached hereto and <br /> incorporated by reference. <br /> • The grant will be paid in increments upon satisfactory achievement of milestones as set forth in Exhibit <br /> "B". All deliverables funded by prior increments must be completed before additional grant increments <br /> will be paid. Any exceptions must be requested and approved by HCF in writing in advance of the <br /> deadline. <br /> • A grant activities report and financial report are required to be submitted to HCF quarterly in order to <br /> administer the grant and maintain necessary expenditure records. Deadlines for the quarterly reports are <br /> set forth in Exhibit "B". <br /> • HCF reserves the right to require site visits while the project is in progress and/or once the project has <br /> been completed. <br /> EXHIBIT "1" <br /> <br />
