~Y~ <br /> HAWAII COMMUNITY FOUNDATION <br /> Exhibit "A" <br /> By accepting this grant, you are obtaining federal funds from the U.S. Department of Justice, Office of <br /> Community Oriented Policing Services (COPS Office). In addition to the Grant Conditions and the attached <br /> Certification and Assurances, your agency agrees to the following grant contingencies: <br /> 1. Use of funds for this grant will adhere to what is "Allowable Costs" under the COPS grants. In addition, <br /> please note the following: <br /> a. Prior approval by the COPS Office will be requested and received for consulting costs over $450 per <br /> day before expenditure. <br /> b. Approval by the COPS federal officer will be requested and received for equipment purchase $5,000 <br /> or more prior to purchase. <br /> 2. The Hawaii Community Foundation has contracted with the University of Hawaii -Center on the <br /> Family to conduct an evaluation of the Hawaii Island Meth Initiative. Your agency agrees to work with <br /> the Center on the Family as it conducts its evaluation. <br /> <br /> 3. Your agency agrees to keep all financial records, including payroll, time and attendance records, <br /> purchase orders, canceled checks, or similar documents associated with your grant for at least three <br /> years from the date of award. <br /> <br /> 4. Participate in training to use web-based reporting system and use the web based reporting system to <br /> submit required quarterly financial reports. <br /> 5. Attend all grantee gathering sponsored by the Foundation for the Hawaii Island Meth Initiative. <br /> <br />
