PUBLIC HOUSING DRUG ELIMINATION GRANT <br /> HHA Agreement <br /> HAWAII COUNTY OFFICE OF AGING <br /> SCOPE OF SERVICE <br /> for <br /> PHDEP "PUKA ANA ... Pushing Through" <br /> Lanakila Homes - Hilo, Hawaii <br /> 1. The CONTRACTOR shall, in a satisfactory and proper manner as determined <br /> by the AUTHORITY, and in strict accordance with the terms and conditions of <br /> this AGREEMENT, provide and perform the services set forth below. <br /> 2. The CONTRACTOR shall provide the services and activities as defined in <br /> Exhibit "A" attached and hereto made a part hereof, and a part of the Scope of <br /> Services of this AGREEMENT. <br /> 3. The CONTRACTOR shall submit reports to the AUTHORITY in the appropriate <br /> formats and within the deadlines specified by the AUTHORITY. Unless <br /> specified, the expenditure reports and progress reports shall be submitted <br /> within thirty (30) days after the end of each calendar month. Should the <br /> CONTRACTOR fail to file the reports with the AUTHORITY on or before the <br /> required date, the AUTHORITY is authorized to withhold funds owed to the <br /> CONTRACTOR until the reports are filed with the AUTHORITY. <br /> 4. The CONTRACTOR shall, within thirty (30) calendar days after the expiration <br /> of the time of performance or a sooner termination date, submit to the <br /> AUTHORITY a final expenditure report, together with any unexpended balance <br /> of funds advanced by the AUTHORITY, and a final report of the services <br /> performed under this AGREEMENT. Should the CONTRACTOR fail to file the <br /> reports with the AUTHORITY on or before the required date, the AUTHORITY <br /> is authorized to withhold funds owed to the CONTRACTOR until the reports <br /> are filed with the AUTHORITY. <br /> 5. The CONTRACTOR shall furnish any additional reports on information that the <br /> AUTHORITY may, from time to time, reasonably require or request. <br /> 6. The CONTRACTOR shall, upon request, meet with representatives of the <br /> AUTHORITY to discuss the progress of the work required thereunder. <br /> 7. The CONTRACTOR shall maintain accounting procedures and practices <br /> acceptable to the AUTHORITY and shall maintain books, records, documents, <br /> and other evidence which sufficiently and properly reflect all direct and <br /> ATTACHMENT 1 <br />
