1995 PHDEP LANAKILA DRUG ELIMINATION GRANT <br /> "PUKA ANA" CONTRACT <br /> HAWAII COUNTY OFFICE OF AGING <br /> Lanakila and Punahele housing units to attend the sessions at the Pomaikai Center. The <br /> number of trips would be 10 trips per day, Monday through Friday for five individuals <br /> per day. <br /> The CSE staff will also conduct a series of community meetings to solicit <br /> residents and community input regarding their specific needs. These community <br /> meetings will be used to establish a positive working relationship between staff and the <br /> residents and provide a one-stop service access for the residents. This will include intake <br /> and assessment, referral and linkages to community services, case management, legal <br /> services and follow-up services to ensure a satisfactory resolution of the problem. <br /> Recreational programs will be incorporated into this program to introduce these <br /> activities as fun and exciting past-time. An intergenerational program will be introduced <br /> where seniors can interact with youth via the computers. <br /> Residents can benefit from on-going training that will encourage socialization and <br /> networking, enable them to gain access to essential resources via audiovisual/technical <br /> computer training and to meet the on-going and future needs of keeping communities <br /> knowledgeable and safe. <br /> Personnel <br /> The Coordinated Services for the Elderly Program (CSE) will assign three (3) <br /> Community Service Worker I positions and one (1) Community Service Program <br /> Assistant position to work directly with the project. The project will additionally be <br /> supervised by the Elderly Activities Division Director and District Supervisor. The <br /> Hawaii County Office of Aging will also provide staff assistance for coordination and <br /> technical assistance efforts. <br /> Evaluation and Data Collection <br /> The Office of Aging will provide on-going monitoring of the program and <br /> provide monthly reports and the activities provided and number of individuals served. <br /> The project's effectiveness will be measured analyzing pre and post nutritional risk <br /> assessment surveys. <br /> Exhibit A - Page 3 of 3 <br />
