No.: X089-7-00-51-5~-H ~ ' <br /> LBR 11I-003 Part-Time Community <br /> Services Employment Opportunities for Older <br /> Workers <br /> STATE OF HAWAII <br /> AGREEMENT FOR GRANTS, SUBSIDIES, AND PURCHASES OF SERVICE <br /> This Agreement, effective the lst of July, 1997, is entered into between the Department <br /> of Labor and Industrial Relations (hereinafter "AGENCY"), State of Hawaii (hereinafter <br /> "STATE"), by its Director (hereinafter "DIRECTOR"), and Hawaii County Office of Aging. <br /> (hereinafter "AWARDEE"), anon-profit organization/governmental agency, under the laws of <br /> the State of Hawaii, whose business address and taxpayer identification numbers are as follows: <br /> 101 Aupuni Street, Suite 342, Hilo, Hawaii 96720. <br /> Federal I.D. No.: 99-6000567; State [.D. No.: 30016002. <br /> RECITALS <br /> A. Chapter 42D, Hawaii Revised Statutes, establishes standards for disbursing public <br /> funds to private organizations to fulfill specified public purposes. <br /> B. The AWARDEE has requested funding from the State of Hawaii for the project or <br /> <br /> program described in Attachment l of this Agreement. <br /> C. The STATE finds that the AWARDEE's performance as described in Attachment t <br /> <br /> of this Agreement will fulfill the public purpose set forth therein. <br /> D. The STATE desires to contract with [he AWARDEE to fulfill the specified public <br /> <br /> purpose, and the AWARDEE is agreeable to performing under this Agreement. <br /> E. Money is available to fund this Agreement pursuant to the Omnibus Consolidated <br /> <br /> Appropriations Act of 1997, P.L. 104-208 and Act 328 of the Session Laws of Hawaii, Regular <br /> <br /> Session of 1997 in the following amount: 5277,165.00. <br /> F. Pursuant to Chapter 42D, Hawaii Revised St<~tutes, and Section 26-20, Hawaii Revised <br /> <br /> Statutes, the STATE is authorized to enter into this Agreement. <br /> NOW, THEREFORE, in consideration of the promises contained in this Agreement, the <br /> <br /> STATE and the AWARDEE agree as follows: <br /> 1. ScQ~ of Perfnrmance_ The AWARDEE shall perform, in a proper and satisfactory <br /> manner as determined by the STATE, the project or program described in Attachment 1, which <br /> is hereby made a part.of this Agreement. <br /> Attach.
