Attachment 1 <br /> SCOPE OF PERFORMANCE <br /> 4rnTP of Srrvirr_ The AWARDEE shall, in a satisfactory and proper manner as <br /> determined by the STATE and in strict accordance with the terms and conditions of this <br /> Agreement, provide and perform the services, activities and programs set forth in the <br /> specifications of the Request for Proposal-Identification Number LBR 111-003, and AWARDEE's <br /> Proposal Application, which are not attached hereto but incorporated as part of this Agreement <br /> <br /> by this reference, and Detailed Program Objectives attached hereto as Exhibit A, and the Budget <br /> attached hereto as Exhibit B. The AWARDEE shall comply with the Cost Principles dated <br /> December, 1994, and applicable federal cost principles listed in OMB Circulars A-87, A-21 and <br /> A-122, which are hereby incorporated and made a part of this Agreement. The AWARDEE shall <br /> ensure that such services, activities, and programs shall be in accordance with Title V of the Older <br /> Americans Act promulgated thereunder; and applicable Federal and State laws and regulations, <br /> policies and instructions. The services, activities, and programs shall be designed to foster and <br /> <br /> promote useful part-time opportunities in community service activities for unemployed, low- <br /> income persons while promoting transition into unsubsidized employment. The AWARDEE shall, <br /> to the maximum extent feasible, cooperate with other agencies providing services to the elderly, <br /> including ITPA agencies. Exhibits A and B supersede previous documents regarding program <br /> <br /> performance and budget. <br /> Perfnrt Banc? <br /> i?PgniremPntc_ <br /> 1. Reporting Requirements: In order that the STATE may monitor the AWARDEE's <br /> compliance with this Agreement, the AWARDEE shall submit written monthly fiscal and quarterly <br /> program progress reports and summary of terminations to the STATE within ten (10) calendar <br /> days after the last day of each calendar quarter during the Agreement period. Quarterly Review <br /> reports are due twenty-five (25) days after the end of each quarter. <br /> 2. Fiscal Reports: The fiscal reports shall detail the uses made by the AWARDEE of <br /> compensation paid to it pursuant to this Agreement, outline the expenditures incurred, and be <br /> certified as to accuracy by the AWARDEE. Such reports are due no later than twenty (20) days <br /> after each month ends. <br /> 3. Program Reports: The program progress report shall consist of statemenu by the <br /> AWARDEE relating to the work accomplished during the reporting period and shall include a <br /> statement of the nature of the work performed, including actual performance measures, activity <br /> levels, and target group indicators, identification of persons served by the AWARDEE during the <br /> reporting period, identification of any immediate problems encountered during the period, and any <br /> recommendations deemed pertinent by the AWARDEE. <br /> 4. Reporting Penalty: Should the AWARDEE fail to file the written fiscal and program <br /> progress reports with the STATE on or before the required date, the STATE is authorized to <br /> withhold funds owed to the AWARDEE until such time as the reports are acceptable and placed <br /> on file with the STATE. <br /> 1 <br /> <br />
