AGING <br />AGING <br />Services Contracted to Private/StateAgencies <br />Case Management. <br />Provide clients served the opportunity to evaluate the program through a survey of <br />which at least 70% of clients surveyed will indicate receiving home and community <br />based services improved their ability to remain in their homes. <br />Information not available at this time. Client surveys are conducted in the <br />second half of the year. <br />Kupuna Care. <br />Provide clients receiving Kupuna Care Services (adult day care, personal care, <br />home -delivered meals, attendant care, chore, home/housekeeper, and assisted <br />transportation services) the opportunity to evaluate the program through a survey of <br />which at least 751/o of clients surveyed will show an improved ability to remain in <br />their own homes. <br />Information not available at this time.
