DEPARTMENT OF THE ATTORNEY GENERAL <br /> Crime Prevention and Justice Assistance Division <br /> INSTRUCTIONS <br /> CERTIFICATION REGARDING DRUG-FREE WORKPLACE REQUIREMENTS <br /> (AG/CPJAD #16) <br /> 1. By signing and/or submitting this application or grant agreement, the grantee, is providing <br /> the certification set out on the form entitled Crime Prevention and Justice Assistance <br /> Division, Departnent of the Attorney General, Certification ReRardingDru~--Free Workplace <br /> Requirements (hereinafter referred to as the "AG/CPJAD Form #16). <br /> <br /> 2. The certification set out on AG/CPJAD Form #16 is a material representation of fact upon <br /> which reliance will placed when the Department of the Attorney General, State of Hawaii <br /> (hereinafter referred to as "grantor") determines to subgrant federal funds to the grantee. <br /> Pursuant to the contract which grantor will offer grantee in the event a subgrant is awarded to <br /> grantee, false certification or violation of the conditions set forth in the certification shall be <br /> grounds for suspension of payments, or suspension or temunation of the subgrant. Such <br /> false certification of violation of the conditions contained in the certification shall subject the <br /> State of Hawaii to govemmentwide suspension or debarment, which shall, in turn, result in <br /> the withdrawal of funds from the gantee and/or the unavailability of future funding for the <br /> grantee. <br /> <br />
