<br /> J , <br /> HOW TO PROCESS A HALE PERMIT <br /> The Hale Permit is a building permit type for an Indigenous Hawaiian Architecture <br /> Structure as adopted in Title 15 Chapter 110 of the Maui County Code. The following are <br /> required for process the Hale Permit. <br /> 1. Completed Building Permit Application for an Indigenous Hawaiian Architecture <br /> Structure: <br /> Part 1 - TMK, street address, property ownership and contact information, <br /> including contact name. <br /> Part 2 - *For completion by DSA. <br /> Part 3 - Required only if permit applicant not the property owner. <br /> Part 4 - Hold harmless agreement, executed by owner/applicant, for <br /> completion by County for permit application number and Mayor's <br /> signature. Caution: Owner(s) signature(s) and notary <br /> acknowledgment to be the same, or it will not be accepted by the <br /> Bureau of Conveyances. <br /> Part 5 - Designate all proposed uses for the selected hale type. <br /> Part 6 - Select only one hale type and size per application. <br /> Part 7 - Select either Class A OR Class B structure. <br /> Note: Class B structures require automatic fire sprinkler <br /> systems. Electrical permit required for fire sprinklers. <br /> Part 8 - Owner builder acknowledgment. ALL blocks to be initialed by <br /> owner. <br /> Part 9 - Acknowledgment for non-exclusive use including use by the general <br /> public: Requires owner's initial and signature; applicant's initials <br /> and signature; licensed contractor's initial, signature, <br /> and contractor information. <br /> Note: Select only part 8 or part 9, not both. Each acknowledgment to <br /> be initialed and signed by owner/lessee, applicant, and licensed <br /> contractor. <br /> Part 10- To be completed by person named on certificate of competency. <br /> Part 11- *For completion by the building inspector. <br /> <br />
