30 Sept 2007 Dear County Council, I am writing you on behalf of the nurses, physicians and health professional who work at the Hilo Community Surgery Center. The lack of access to medical care on the Big Island has been labeled a major health care crisis by the press and many of our political leaders. Many patients in East Hawaii have difficulty in attending appointments with surgeons, orthopedic specialists, OB-GYN and women's specialists. The Community Surgery Center was established to help meet these critical medical shortages, in the area of Hilo, which has the largest concentration of medical practices. The lack of adequate parking is seriously limiting the ability of East Hawaii patients to seek medical care. Our health professionals are dismayed to see women in their late stages of pregnancy walk blocks to reach their appointments. Elderly women often have to park far away on the street or in other distant lots, and trudge through the rain to have mammography. The lack of adequate parking is a major roadblock to medical care in Hilo, especially for those with limited mobility. The Community Surgery Center has a request before the County Council to establish more parking for our patients. It is impractical to develop parking elsewhere in town and bus our patients in. Hawaii is already a very challenging place to operate a medical practice. Business expenses are high, reimbursements are low and it is extremely difficult to recruit and retain medical professionals. If patient access problems push health care specialists out of their practices at the Community Surgery Center, it would be difficult to find replacements, especially for surgical and OB-GYN doctors. The additional parking is also needed for the new physicians moving into the Community Surgery Center and for women who need mammography. Thousands of women in East Hawaii require annual mammography, as the number of mammograms performed actually fell between 2002 and 2005, despite the increase in the population. The only mammography clinic in East Hawaii is located at the Surgery center and is willing to invest in very expensive digital mammography equipment to shorten exam time to 10 minutes and enable same day mammogram appointments. However, this modern and more efficient mammography equipment cannot screen the additional thousands of women who need mammography without more parking for them to reach the clinic. Comm. The American Cancer Society believes that, after tobacco use, lack of access to quality health care in the United States could be the biggest barrier to continued progress in the fight against cancer. Most solutions, like a new family practice residency, are years in the future and would not address the currently existing shortage of specialists. The single most important step the County Government could take would be to facilitate adequate parking so patients can see their current health care providers. We understand that there are residential homes in our neighborhood, and are very willing to work toward an accommodation that will enable the needed parking and also address their concerns. As the Director of the only mammography clinic in East Hawaii, please understand what is at stake here. As current President of the Community Surgery Center, I have already asked our staff to park off site. Unlike the County building where workers have reserved parking, our medical staff park on streets outside the centers, which means patients who do cannot find parking have to park blocks away. Additional parking for patients is desperately needed. The Planning Commissioner and the Planning Commission have already approved the additional parking. It is logical and appropriate to vote in favor of rezoning to provide better access to health care in Hilo. Council members voting in favor of rezoning will send the right message to other health care providers who wish to expand and develop critically needed medical services on the Big Island. Doctors whom try to improve or develop medical services here should be supported, not demonized, opposed and criticized. Tens of millions of Americans live next to parking lots. In metropolitan areas in the U.S., including Honolulu, almost every public and private housing complex is situated next to parking lots. I have lived next to parking lots most of my life, and live next to one now. The intent is to have this patient parking lot open during daylight hours and locked in the evenings and weekends. Obviously, a better job should have been done in the past to meet the legitimate concerns of the neighbors, such as better privacy barriers or greenbelts. However, it is both insulting and silly to think that either patients or health care professionals are going to run down to the parking lot to commit crimes during the day. All of us need health care, and it is unreasonable for us to insist on health care for ourselves, but deny access to quality health care for our fellow citizens. Council members who oppose rezoning should understand what their action means. It is simply impractical to insist that patients park offsite and be forced to be bused in to the medical clinics. Patients are already enduring long waits in doctors offices because the shortage of doctors. If forced to endure additional waiting for buses many will simply stop seeking medical care and flood already overburdened emergency rooms. Doctors are small businessmen and women who already have a hard time making ends meet in Hawaii. As a recent Chief of Staff at Hilo Medical Center, I can ask two other respected doctors who served in that position explain how their practices are barely profitable. One of these critically needed specialists is closing his medical practice this month. My own primary care physician shared with me that he had already declared bankruptcy twice and finally decided it was better to retire than lose money practicing medicine in Hawaii. If the Council disregards the recommendations of the Planning Director and Planning Commission Council and opposes the rezoning of the existing parking lot, then I will be forced to recommend that the Community Surgery Center build a large highly expensive parking structure on our current parking lot to meet the needs of East Hawaii patients. Access to medical care will actually become much worse for many months during construction. Many hundreds of thousands, or even millions of dollars, that could have been spent on new surgical technology, digital mammography and newer imaging equipment and additional medical staff, will instead be spend on concrete for an elevated ramp which will tower over the neighborhood. In effect, a huge tax will be placed on the health care providers at the Surgery Center. The costs will inevitably have to be passed on the patients, or additional providers will go out of business, retire early or leave the Big Island. More unfortunately, the lack of access to mammography, cervical cancer screening with OB-GYN providers, etc, will take it's toll in higher mortality for patients in East Hawaii.
