~f,~(Y or y~_ <br /> COUNTY OF HAWAII STATE OF HAWAII <br /> •••rI'p~'~11~+ <br /> RESOLUTION NO. 512 OS <br /> <br /> A RESOLUTION TRANSFERRING/APPROPRIATING AN APPROPRIATION OUT <br /> <br /> AND FROM THE DESIGNATED FUND ACCOUNT(S) AND CREDITING SAME TO <br /> A DESIGNATED FUND ACCOUNT(S) <br /> WHEREAS, the Bay Clinic, Inc has requested funds to conduct a Diabetes Self <br /> Management Education Program; and <br /> WHEREAS, Bay Clinic, Ina (BCI) is a nonprofit community health centers <br /> providing low-cost health care to the uninsured, underinsured and low-income residents of <br /> East and South Hawaii Island; and <br /> WHEREAS, the program is geared towards helping patients control the negative <br /> health effects of diabetes in the aging native Hawaiian communities in Hilo and Keaukaha; <br /> and <br /> WHEREAS, Contingency Relief Funds from Council District 4 will be appropriated <br /> to the Office of Aging to be used towards the Diabetes Self Management Education Program; <br /> now, therefore, <br /> BE IT RESOLVED BY THE COUNCIL OF THE COUNTY OF HAWAII that <br /> the following amount is hereby transferred appropriated as set forth below: <br /> FUND: General <br /> AMOUNT OF <br /> APPROPRIATION: $10,000 <br /> OUT AND FROM: <br /> 010.101.5101.91 Clerk-councilSVC-Contingency Relief $10,000 <br /> CREDITED TO: <br /> 010.411.5411.10 Office of Aging $10,000 <br /> Area Plan On Aging OCE <br /> (Diabetes Self Management Education <br /> Program) <br /> <br />
