Home Modification. <br /> Provide limited funding to eligible older individuals who have been assessed with <br /> impairment or limitations to perform Activities of Daily Living/Instrumental <br /> Activities of Daily Living. Enable participants to live safer within their own home <br /> and help to correct safety deficits. At least 75% of the clients served have favorable <br /> responses that in-home safety items and/or adaptations allowed them to remain within <br /> their homes in a safer environment due program services. <br /> <br /> During the period 07/01/07 to 12/31/07, 109 older individuals were served with <br /> 295 units of home modification. A survey will be conducted of clients served in <br /> June 2008. <br /> 7. Healthy Aging Initiative. <br /> Support and coordinate physical activity(ies) at selected congregate dining site(s) <br /> where at least 75% of participants who are screened and surveyed will indicate that <br /> the physical activity(ies) improved or maintained their health status. <br /> <br /> The Enhance Fitness project was terminated for various reasons. <br /> Currently Office of Aging is collaborating with Ain Like to provide a Chronic <br /> Disease Self Management Program at its nutrition sites utilizing the same federal <br /> funding source. <br /> Services Contracted to Private/State Agencies <br /> <br /> 1. Case Management. <br /> Provide clients served the opportunity to evaluate the program through a survey of <br /> which at least 70% of clients surveyed will indicate receiving home and community <br /> based services improved their ability to remain in their homes. <br /> <br /> Annual survey done in April of each year. <br /> 2. Kupuna Care. <br /> Conduct reassessments of Kupuna Care clients of which at least 70% of all clients <br /> that are discharged from the program will have maintained or improved their <br /> Activities of Daily Living (ADL) or Instrumental Activities of Daily Living (IADL) <br /> levels. Kupuna Care services include adult day care, personal care, home-delivered <br /> meals, attendant care, chore, home/housekeeper, assisted transportation, and Kupuna <br /> Care transportation <br /> <br /> During the first six months twenty-four discharge reassessments were randomly <br /> selected for review on Kupuna Care clients. Out of twenty-f our clients, nineteen <br /> or 79% maintained or improved their ADL or IADL level of impairment.
