The Division of Water Resource Management (DWRM), Geology-Hydrology <br /> Section, shall be notified at S48-7543, before any work covered by this <br /> permir commences. <br /> <br /> 2. The proposed use shall not adversely affect existing legal uses in the area. <br /> <br /> 3. The grouted annulus of the well' shall be from 0 to 500 ft. instead of from 0 <br /> to 50 ft. as proposed, and the casing shall be designed accordingly. <br /> <br /> 4. The following shall be submitted to DWRNI, P.O. Box 373, Honolulu, Hawaii <br /> 96809, within 30 days after completion of the well: <br /> <br /> a. Well Completion Report. <br /> <br /> Ii. Elevation (referenced to ocean sea level) survey by a Hawaii-licensed <br /> surveyor. <br /> <br /> c. As-built sectional drawing of the well. <br />
