Give reasons how new duties arose (statutes enacted, ordinances, newly <br /> authorized programs, basis for increased workloads, etc.): <br /> <br /> The Office of Aging's Caregiver Resource Center is now an integral component <br /> of the Aging & Disability Resource Center (ADRC). The position is needed to <br /> coordinate a system of care to assist older adults and their caregivers maximize <br /> their quality of life and to manage their health and activities of daily living with <br /> appropriate and efficient support. The position will also provide much needed <br /> options counseling at the one-stop facility. <br /> <br /> 2. Can new duties be assigned to existing positions or handled in other way such as <br /> short term contracts, etc. <br /> <br /> Although able to assist, planners within the Office of Aging would not be able to <br /> take on such a comprehensive as well as time consuming function. <br /> <br /> 3. How will new position increase the efficiency/effectiveness of operations? (Use <br /> productivity measurements where possible.): <br /> <br /> As caregivers will represent the bulk of clients being served through the ADRC, <br /> the position will play a very important role in addressing their needs in an efficient <br /> and effective manner. Management and follow up of caregiver needs and <br /> services will certainly enhance the efficiency and effectiveness of the program. <br /> Coordination of care includes such responsibilities as information and assistance, <br /> outreach, screening, assessment, service authorization, ordering and delivery of <br /> services, coordination and follow up, and evaluation of data and services. <br /> <br /> <br /> <br /> <br /> DCS CP-002 (2/93) <br />
