DUTIES OF THE POSITION <br /> List each duty assigned to the position separately. If mores ace is needed, use a blank sheet and attach to this form. <br /> <br /> Position is responsible to coordinate a system of care to assist older adults and their caregivers maximize <br /> Their quality of life by providing access assistance to appropriate services, information on available community <br /> Resources and information and training which positively impacts and supports their caregiving situation. <br /> A. Program Development, Coordination and Implementation 60% <br /> <br /> 1) Provides leadership, information and assistance, and coordination of care for caregivers. (1) <br /> a. Responsibilities include provision of services to individual caregivers and to groups of caregivers <br /> which include: information, assistance, outreach, screening, assessment, coordination of care <br /> and follow up. (1,3) <br /> b. Develops training materials on topics of interest to caregivers and provides training and workshops. (2) <br /> c. Evaluates assessments and develops a care plan with the caregiver and assessor within <br /> predetermined budget parameters. (2) <br /> d. Coordinates and monitors the activities identified in the care plan. (1,2) <br /> e. Serves as a member of the Case Management Coordination Project Interdisciplinary Teams. (1) <br /> 2) Authorizes and coordinates vendor services requested by assessors. <br /> a. Orders or approves the ordering of appropriate, approved services. (1,2) <br /> b. Coordinates the delivery or services to caregivers. (1,2) <br /> c. Manages the purchase of services to ensure costs are within approved budgetary guidelines. (1,2) <br /> d. Maintains budgetary and program records to evaluate program effectiveness. (1,2) <br /> e. Approves all invoices for vendor services and submits to fiscal officer for processing and payment.
