BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED): <br />The Hawaii County Office of Aging was awarded grant funds from the Administration on Aging via the State <br />Executive Office on Aging to coordinate the delivery of evidence-based prevention through an aging service <br />provider organization.
