No. 712-08 <br /> PLACE: Sheraton Keauhou Bay Resort & Hotel <br /> 78-128 `Ehukai Street <br /> Kailua-Kona, Hawaii <br /> TIME: 9:30 a.m. <br /> <br /> Council Chair and Members <br /> Hawaii County Council <br /> Hilo, Hawaii 96720 <br /> <br /> Your Committee on Finance, to which was referred Resolution No. 712-08, reports as follows: <br /> <br /> Resolution No. 712-08, transmitted by Director of Finance William Takaba, via Communication <br /> No. 1359, dated July 24, 2008, amends transfers funds as follows: <br /> <br /> From: <br /> 010.481.5483.15 Healthy Aging Partnership (Nutrition Program) General $31,834.34 <br /> <br /> To: <br /> 010.411.5411.93 Healthy Aging Partnership (Office of Aging) General $31,834.34 <br /> <br /> The County of Hawaii was awarded grant funds from the Administration on Aging to coordinate <br /> the mobilization of the public and private partnerships at the community level to implement and <br /> sustain the delivery of evidence-based prevention through an aging service provided <br /> organization. The Hawaii County Office of Aging (HCOA) contracted with the Hawaii County <br /> Nutrition Program (HCNP) to implement Enhance Fitness, an evidence-based exercise program, <br /> in two nutrition sites, which for a number of reasons Enhance Fitness did not prove to be a very <br /> good program for the nutrition sites, and a difficult decision to terminate the program was made. <br /> <br /> The HCOA is committed to maintaining and/or improving the health status of elderly citizens, <br /> and has requested approval from the State Executive Office of Aging (EOA) to explore the <br /> possibility of implementing the Stanford University Chronic Disease Self Management Program, <br /> which is another grant, approved evidence-based prevention. The EOA granted its approval, and <br /> HCOA will use the unspent funds which will be transferring out and from the Elderly Activities <br /> Division's Nutrition Program's Healthy Aging Partnership account, and crediting it to the <br /> Hawaii County Office of Aging's Healthy Aging Partnership account, to contract with another <br /> aging service provided organization. <br />
