[Document C, 2 of 5 pages] <br />Uluakupu Awards — Community Grants Program <br />Executive Summary <br />• Name of organization project title (if applicable), amount requested: <br />BIG ISLAND TASK FORCE ON CRIMINAL JUSTICE: 704 Handbook Committee, Hawaii County <br />Amount Requested: $24,999.00 <br />• Brief description of your organization and its mission: <br />We represent the collaborative'efforts of multiple agencies (Judiciary;•Police;^Prosecutors,°Public'- <br />Defenders, Probation Department, Department of Health, and Adult Mental HealthDivision) to <br />improve the outcomes for severe and persistent mentally ill (SPMI) offenders. <br />Mission:.Transforming and elevating the awareness, ability and response of the people who <br />interact professionally with SPMI consumers. <br />• Description of project or program, goals, planned activities and strategies: <br />1) Evaluation and assessment of current responses: <br />a) Identification of current issues as seen by responders and SPMI consumers <br />2) Plan of action based on assessment. <br />3) Training for supervisory and line staff: <br />a) Initial Training/Workshops. <br />b) Quarterly Surveys (follow ups — feedback on how this has improved the participant's work <br />with SPMI consumers, participant recommendations to create a feedback loop for future <br />training and workshops). <br />c) Follow -Up Annual Trajnings,incorporating the feedback and issues brought up by Quarterly <br />Surveys throughout the year. <br />4) Development of collaborative relationships amongst participants. <br />5) Development •of One and,Three-Year plans to address.problems and maintain gains ..r.I <br />• Description how project or program addresses one or more of the President's New Freedom <br />Commission Goais/Hawaii s CMHP recommendations or MHT SIG Task Group Goals: <br />CMHP-SWG#1: Promoting and Understanding Mental Health <br />1.1 Design and implement a public education campaign to promote the understanding, of <br />mental health issues. <br />Advance and implement a campaign to improve the response to SPMI consumers on jail diversion or <br />conditional release status. <br />CMHP-SWG#4: Accelerating and Expanding Quality Services: <br />4.2 Establish a system that makes sure that all providers are doing what works best.
