This is a summary of the Jail Diversion clients <br />served since 2003 until August 2008. <br />The Office of Social Ministry formed a transitional housing program in Hilo, called Ponohawai'ola. This <br />transitional housing provides support for inmates returning to the community from incarceration. <br />During the first year, half of the people who went through the program were determined eligible for <br />AMHD services. During the current year, 67% of the returning inmates were found eligible for AMHD <br />services. It is not known if these people were placed in prison and the experience of being in prison <br />kindled an already existing SPMI diagnosis or if they could have been made eligible for AMHD services <br />prior to arrest or sentencing. However, it underlines the importance and need forjail diversion and <br />conditional release activities. <br />Describe the specific work for which funding is being requested and what problems, needs or <br />opportunity this request addresses. Clearly identify the goals and objectives. <br />Description of project or program, goals, planned activities and strategies: <br />• Evaluation and assessment of current responses (baseline data) — by end of 2008 calendar year. <br />• Identification of current issues as seen by responders and consumers <br />• Plan of action based on assessment — by end of January 2009. <br />• Training for supervisory and line staff: <br />initial Trainings — May 2009 <br />Follow -Up Annual Trainings addressing feedback and issues brought up by Quarterly <br />Surveys conducted quarterly. <br />• Quarterly Surveys (follow ups —feedback on how this has improved the participant's work with <br />the SPMI consumer, participant recommendations to create a feedback loop for future training <br />and workshops) August, November, February, and March (for life of project) <br />• Development of collaborative relationships amongst participants — these professionals include: <br />Judges, attorneys, police, parole and probation staff, correctional center staff, first responders, <br />hospital staff, adult protective services staff, mental health professionals. <br />• Development of One- and Three -Year plans to address problems and maintain gains. <br />4. Identify the activities to accomplish the objectives and the goals. Assign a GPRA (Government <br />Performance Result Act) indicator, if applicable. <br />GPRA Indicator #2: Increase the number of persons in the mental health care and related workforce <br />who have been trained in service improvements recommended by the CMHP. <br />Goal: Our goal is to create an organic process by which people that do not treat SPMI consumers, but <br />in the course of their work may interact with them, become more knowledgeable and sensitive to <br />mental illness and the related stigma. <br />
