Aging and Disability Resource Center Development Project <br /> Provide older adults and caregivers information and assistance services.. <br /> a. At least 15% of population served will be connected to appropriate service(s) and <br /> resources. <br /> <br /> The Aging & Disability Resource Specialist, Caregiver Specialist and ADRC <br /> Coordinator are increasingly the primary responders to walk-in and <br /> telephone inquiries to the Office of Aging as the face of the office shifts to an <br /> Information and Assistance model. Individuals contacting the office with <br /> Long Term Care and community support concerns meet with trained and <br /> experienced staff to address their needs. In excess of 15% of seniors, <br /> caregivers and individuals with physical disabilities are connected to <br /> appropriate services and resources. <br /> <br /> b. At least 75% of population served will make appropriate decision(s) as a result of <br /> information and assistance received. <br /> <br /> Care decisions are difficult ones as they often impact finances and lifestyles <br /> changing familiar settings and routines. Decisions change as presenting <br /> needs shift and also as individuals and families act on options. At least 75% <br /> of individuals and families make informed decisions based on options <br /> information made available through counseling. <br /> .2. Caregiver Support: National Family Caregiver Support Program <br /> a. Develop and maintain information and outreach displays and presentations about <br /> the availability of support services in the community through public education in <br /> group settings (health fairs, community service fairs, or community organization <br /> meetings). <br /> <br /> In the first six months of the fiscal year, the Caregiver Support Program <br /> participated in four events providing information in person, in displays and <br /> with hand out materials to interested caregivers and the general public. <br /> <br /> July 9 Caregiver Respite Day <br /> October 7 Queen Liliuokalani Children's Center Grandparent <br /> Support Group <br /> November 18 Primetime Wellness Fair <br /> November 19 Hoike Club (UE Women's Club) <br /> <br /> <br /> <br /> <br /> Six Month Progress Report on Program Objectives for FY 2008-09 1 <br />
