shall subject the State of Hawaii to government -wide suspension <br /> or debarment, or other sanctions which, in turn, shall result in <br /> the withdrawal of funds from Grantee and /or the unavailability <br /> of future funding for Grantee. <br /> 5. Prior to, or concurrently with the execution <br /> of this Contract, Grantee shall complete, execute and submit to <br /> Agency a Certification Regarding Debarment, Suspension, <br /> Ineligibility and Voluntary Exclusion, hereinafter referred to <br /> as the "Debarment Certification." A copy of the Debarment <br /> Certification shall be included in Part IV of the Application <br /> for Grant. Grantee covenants that the representations made in <br /> the Debarment Certification are true at the time this Contract <br /> is executed and will remain true throughout the entire term of <br /> the Contract and any extensions, and that Grantee shall fulfill <br /> any and all terms and conditions set forth therein. <br /> 6.
