Timeline: 711/11— 6130/12 <br />5) Provide consumer with housing referrals to acquire a place to live. This <br />will increase their independence and decrease dependency on a family <br />and /or friends. Timeline: 7/1/11— 6/30/12 <br />6) Provide consumers wilt attendant referrals to acquire personal care <br />assistance services. This will increase their independence to remain at <br />home versus institutiorranalization. This will increase self -worth and self- sufficiency <br />in their dais living. Timeline: 6/1/11 - 6/30/12 <br />Program Highlights: (from Fiscal Year 2009 -10) <br />1) Established a fully functioning board of directors, 51 % of whom are <br />persons with disabilities. <br />2) Hired two Independent Living (IL) Specialists that assists the Executive <br />Director in direct consumer servies. <br />3) Collaborated with the following organizations: Kahuku Medical Center to <br />facilitate services for the frail and elderly to maximize existing resources <br />and to prevent premature and/or inappropriate institutionalization. <br />Ho'oikaika Hawaii Peer Mentioring Project for Persons with Traumatic <br />Brain Injury to identify consumers with traumatic brain injury and facilitate <br />appropriate services based on their needs that will assist with their <br />independence. U. S. Army Wounded Warrior Program to assist with <br />soldiers transition needs for living in the community and maintaining their <br />independence. Hawaii Fi -Do Service Dogs in developing a donation <br />program to provide essential dog care items for consumers who own a <br />service, companion or therapy dog. <br />4) Organized a youth recreational group to provide recreational opportunities <br />for Oahu's North Shore youths while providing independent living skills <br />such as cooperation, responsibility, self - advocacy and interpersonal <br />realations. <br />NONPROFIT GRANT APPLICATION <br />FISCAL YEAR 2011 -12 <br />Page 3 <br />
