AGING <br />AGING <br />d. Implement a formalized protocol and resource materials for options counseling <br />for individuals, families and caregivers. <br />The state Executive Office on Aging was unsuccessful in its Options <br />Counseling grant application which would have provided a statewide model. <br />In the absence of this resource, a county based model will be developed until <br />a statewide model is adopted. <br />e. Develop a Memorandum of Agreement with at least one acute hospital facility to <br />streamline Aging and Disability Resource Center referrals and service access for <br />eligible individuals. <br />The statewide Hospital Discharge Planning grant workgroup is nearing <br />selection of a model to implement with targeted hospitals. As this model is <br />finalized a Memorandum of Agreement will be developed with a target <br />hospital in Hawaii County. The Memorandum of Agreement will describe a <br />referral process and services to be extended by the Aging and Disability <br />Resource Center. The Memorandum of Agreement is targeted to be on place <br />by June 2011. <br />f. Fully implement Beacon IR tracking software for all I &A contacts with the Aging <br />and Disability Resource Center <br />The Social Administration Management System/ Information and Referral <br />program is fully implemented as the data and tracking software. The Social <br />Administration Management System/ Information and Referral program <br />incorporated the intake features of the Beacon Information and Referral <br />program with Hawaii County Office of Aging's database in the Social <br />Administration Management System program. <br />2. Caregiver Support: National Family Caregiver Support Program <br />a. Develop and maintain information and outreach displays and presentations about <br />the availability of support services in the community through public education in <br />group settings (health fairs, community service fairs, or community organization <br />meetings). Participate in at least seven (7) outreach sessions in a variety of <br />settings.
