AGING <br />AGING <br />3. Community Planning and Development — Partnerships <br />Healthy Aging Initiative <br />a. Increase by 20% the number of chronic disease self management workshops <br />offered in Hawaii County. The 20% increase would be based on the number of <br />workshops completed for the fiscal year ending June 2009. The intent is to <br />embed a health related evidence -based program in Hawaii County. <br />For the FY ending June 2009, 6 chronic disease self management workshops <br />were completed. For the 6 month period ending December 2010, 7 self <br />management workshops were completed; 6 chronic disease self management <br />workshops serving 40 elderly persons and 1 arthritis self management <br />workshop serving 11 persons. The workshops were conducted in Hilo, Ka`u, <br />Kona, and Waimea. <br />b. Support and coordinate an evidence -based community program where at least <br />75% of participants surveyed will indicate that the evidence -based community <br />program improved or maintained their health status. <br />As part of the chronic disease self management program, each participant <br />completes a follow -up survey 6 months after each workshop to check on the <br />health status of the participants. The results of these surveys will be <br />available by June 2011. <br />4.
