Parker <br />Executive on Aging <br />min <br />P4 <br />During these challenging times, the entire Aging Network from the federal level down to <br />the state and local levels are undergoing considerable change as the entire country struggles to <br />address ever increasing health care costs accompanied by an exploding elder population. As part <br />of this national effort, Area Agencies on Aging across the country are embarking on various <br />"systems change" initiatives to position themselves to be eligible for expanded or redirected <br />federal dollars geared to promote efficiency and cost savings especially in the area of Long Term <br />Care Supports and Services. <br />The Office of Aging, the designated Area Agency on Aging for Hawaii County, is <br />currently working very closely with our Aging Network here on the Big Island, the State <br />Executive Office on Aging, the other three counties, and the U.S. Administration on Aging to <br />implement the Hawaii Systems Change Initiative that involves three large grants: the Aging and <br />Disability Resource Center (ADRC) grant; Community Living Program (CLP) grant; and the <br />Person - Centered Hospital Discharge Planning (HDP) Initiatives. The State Executive Office on <br />Aging is also working very closely with the State's Long Term Care Commission established by <br />the State Legislature in 2008 through Act 224 charged with identifying needed reforms of the <br />long -term care system, researching program changes and resources needed to meet the state's <br />long -term care goals, and exploring funding options that may help support the provision of long- <br />term care services in the State of Hawaii. <br />During the week of December 5 to 9, 2011, Steven Lutzky, PhD, President of HCBS <br />Strategies Inc., the agency contracted by the state Executive Office on Aging (EOA) to <br />implement a systems change effort in the State of Hawaii will be here and is available to make a <br />presentation to the County Council on the efforts being pursued. I would like to request putting <br />Mr. Lutzky on the Human Services, Social Services and Public Safety Committee agenda for his <br />presentation that will take approximately 30 minutes. <br />Hawaii County is an equal opportunity provider and employer. <br />An Area Agency on Aging <br />
