REQUEST: <br />To increase $30,000 in revenue account 3301.15 Coordinated Services <br />Also to increase the corresponding expenditure account 010.481.5481.03.454 CSE - Eqpt by $30,000 <br />BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED): <br />The State Office of Aging has received federal funds to assist with the Sage Plus Program. The program will <br />assist the Hawai'i State Health Insurance Program (SHIP) in providing members with medicare for face to <br />face comparison and enrollment in Medicare Health and Drug plans through community events and one to <br />one counseling as well as maximize untilization of existing community services and resources. As <br />Coordinated Services is the current contractor for the HCOA Outreach services to provide older adults free <br />assistance with Medicare paperwork, CSE has been awarded $30,000 to purchase computers and <br />accessories. <br />SIGNED: <br />Department Head <br />DATE: / I(f' �l <br />
