Attachment <br /> CONTRACT MODIFICATION: <br /> Effective February 29, 2012, the parties mutually agree that the PROVIDER shall continue the <br /> provision of services with the following modification(s): <br /> 1. Attachment 3, Compensation and Payment Schedule. <br /> a. Modification Order No. 2. <br /> The New Adjusted Contract Price in Section C. is changed from"$1,266,841.12" <br /> to "$1,226,841,12"to correct a math subtraction error. <br /> b. The total amount of compensation is increased by "$1,008,428.21"of federal <br /> funds for fiscal year 2012 from "$1,226,841.12"to"$2,235,269.33." The <br /> "Budget Summary,"attached to the Contract as Exhibit"I,"is hereby deleted and <br /> replaced with a"Budget,"attached hereto as Exhibit"J"and made a part hereof. <br /> In addition, the PROVIDER shall submit a STATE approved detailed Budget no <br /> later than fifteen (15)days after the execution of this modification, and failure to <br /> comply may result in the withholding of payments to the PROVIDER, Upon <br /> submission of a STATE approved detailed Budget, the STATE approved detail <br /> Budget shall become a part of Exhibit "J" and made a part of this Contract, <br /> 2.
