BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED): <br /> Grant funds (Diabetes prevention and Control Program, DOH) awarded to Hawaii County Office of Aging to <br /> continue implementation of the Chronic Disease Self Management Program (CDSMP) by facilitating and <br /> implementing training of Lay Leaders for the Diabetes Self Management curriculum (DSMP).
