Form #: B-52
7/18/91
DEPARTMENT OF FINANCE
REQUEST FOR COUNCIL ACTION
DEPARTMENT: FIRE DATE: January 16, 2013
STAFF CONTACT: Lance Uchida, Battalion Chief, EMS PHONE: 961-8319
A. REQUEST:
Please prepare a resolution to accept a monetary donation of$3,500 from Kaiser Permanente and send a
letter of appreciation to:
Nina Miyata
Kaiser Permanente Hawaii, Community Benefits
2828 Paa Street
Honolulu, HI 96819
B. BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED):
This donation was a result of a request submitted from the Hawai'i Fire Department (HFD), EMS Bureau to
the Kaiser Foundation Sponsorship and Donation program. Our submission was for funding to purchase
ruggedized digital cameras for the purpose of taking necessary photographs at the scene of an emergency,
most specifically for documenting the mechanism of injury (MOI) at the scene of a motor vehicle crash
(MVC).
Photographs documenting the MOI have become a standard best practice in the EMS industry as there is
tremendous assessment and treatment benefit for the emergency department staff by viewing the photo's
from the scene of an MVC. HFD had for years practiced this by way of instant Polaroid photos. However
Polaroid stopped production of the film in 2008. Cameras will be placed on each HFD EMS Unit.
JAN 17 2013
DATE:
Department Head
