To maintain and foster collaborative relationships with relevant eldercare, <br /> community based private and public agencies including those working with the <br /> disabled population of all ages; <br /> 9. To promote new or expanded benefits and opportunities for older persons; <br /> 10. To develop and administer an Area Plan on Aging for a comprehensive and <br /> coordinated service delivery system in Hawai`i County; <br /> 11. To enter into sub-grants or contracts for the provision of services outlined in the <br /> Area Plan on Aging; and <br /> 12. To provide technical assistance, monitor, and periodically evaluate the performance <br /> of all service providers under the Area Plan on Aging. <br /> Program Objectives <br /> Empower older adults to stay healthy, active, and socially engaged using prevention <br /> and disease self-management techniques. <br /> 1. Community Planning and Development/Healthy Aging Initiative will continue to <br /> expand the Better Choices, Better Health—Ke Ola Pono Program, a chronic disease <br /> self-management evidence-based community program, to the districts of Puna <br /> (Mauka and Makai) and Kohala(North and South). <br /> a. A minimum of 2 workshops serving 16 participants in each district for the fiscal <br /> year ending June 30, 2015. <br /> b. Build capacity of the Better Choices, Better Health—Ke Ola Pono Program, in the <br /> Kohala and Kona districts. Conduct Lay Leader trainings in each district <br /> graduating 12 Lay Leaders for the fiscal year ending June 30, 2015. <br /> Enable older adults to remain in their homes with a high quality of life for as long as <br /> possible through the provision of home and community based services, including <br /> supports for caregivers thus preventing and/or delaying the need for institutional care. <br /> 3 <br />
