Cooperative Agreement P 14AC00084 <br /> Page 2of16 <br /> ARTICLE III—STATEMENT OF WORK <br /> A. The NPS agrees to: <br /> 1. Provide for the period of this agreement a modern type-1 ambulance to <br /> HFD through its lease from the General Services Administration, <br /> including gurney and emergency radio meeting HFD specifications. This <br /> lease will provide for the maintenance, fuel, oil, and general repair of the <br /> ambulance until the allocated funding of$30,000 is depleted. <br /> 2. Provide management oversight. <br /> 3. Meet with HFD on a regular basis to discuss operational procedures. <br /> B. HFD agrees to: <br /> 1. Provide ambulance services with trained paramedic staff to treat visitors <br /> and employees within the Park. <br /> 2. Stock the NPS-provided ambulance with basic and advanced life support <br /> equipment needed to provide emergency care. <br /> 3. Provide both basic and advanced life support emergency care inside the <br /> Park and the surrounding communities. <br /> 4. Reimburse Hawaii Volcanoes National Park for any expenditures above <br /> $30,000 for the ambulance lease. <br /> ARTICLE IV—TERM OF AGREEMENT <br /> The Agreement will become effective upon the date of the last signature and will expire <br /> five years from that date, unless terminated earlier per Article XI. The period from the <br /> effective date to the expiration date is the period of performance for the Agreement. <br /> ARTICLE V— KEY OFFICIALS <br /> A.
