Attachment <br /> CONTRACT MODIFICATIONS: <br /> Effective October 15, 2015, the parties mutually agree that the CONTRACTOR shall <br /> continue the provision of services of the Contract with the following modifications: <br /> 1. Attachment-3, Compensation and Payment Schedule <br /> a. The total amount of compensation is increased by$35,000.00 of federal funds <br /> which shall be paid in accordance with provisions of the Compensation and <br /> Payment Schedule. <br /> b.
