Transport all waste tires collected from residential communities to Department of Health— <br /> ("DOH")permitted or out-of-state recycling facilities by July 31, 2016. The quantity of tires <br /> transported and recycled under this Contract shall be limited to those collected between <br /> November 6, 2015 and May 31, 2016, and as part of a mosquito control program. <br /> 2. Maintain all transport and recycling documentation, including receipts from the recycler, to <br /> ensure that all collected waste tires are recycled at DOH-permitted or out-of-state recycling <br /> facilities. <br /> 3. Expend funds under this Contract to pay for the costs of materials, equipment and/or <br /> subcontractors required to transport and recycle residential waste tires collected between <br /> November 6, 2015 and May 31, 2016, and as part of a mosquito control program; and transported <br /> by July 31, 2016 to recyclers. <br /> 4. Submit to the STATE's Office of Solid Waste Management a written report of the services <br /> performed under this Contract by August 31, 2016. The report shall include,but not be limited <br /> to: <br /> a. A summary of the total number of waste tires collected from residential communities <br /> between November 6, 2015 and May 31, 2016, and the general locations from which <br /> these tires originated; <br /> b. The dates and locations to which the waste tires were transported for recycling; <br /> c. An expenditure report listing expenses incurred for the transportation and recycling of the <br /> waste tires collected during the specified period; <br /> d. Copies of pertinent receipts from subcontractors; and <br /> e. Motor Vehicle Tire Recovery Summary, OSWM-T2, attached hereto as Exhibit "B" and <br /> made a part of this Contract. <br /> 5. Prevent, treat and manage any collection of standing or flowing water to control the breeding of <br /> mosquitos at county facilities where tires are stored in accordance with Hawaii Revised Statutes <br /> ("HRS"), Section 321-11(1); Hawaii Administrative Rules Chapter 11-26, Subchapter 3; and all <br /> subsequent amendments; and <br /> 6. Comply with all applicable state laws regarding environmental quality, including Chapters 342B <br /> through 342P, HRS, and their federal counterparts. In the event that a state or federal agency <br /> determines that the CONTRACTOR or its subcontractor has violated any state or federal law <br /> regarding environmental quality, the STATE may terminate this Contract after providing thirty <br /> (30) calendar days notice. <br /> ADM.
