NO. <br />A� BILL FOR AN ACT <br />RELATING TO MOTOR VEHICLE DRIVER'S LICENSES AND CIVIL <br />IDENTIFICATION CARDS. <br />BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF HAWAII: <br />1 SECTION 1. Section 286-111, Hawaii Revised Statutes, is <br />2 amended by amending subsection (a) to read as follows: <br />3 "(a) Every application for an instruction permit, <br />4 provisional license, driver's license, limited purpose driver's <br />5 license, limited purpose provisional driver's license, or <br />6 limited purpose instruction permit shall be made upon a form <br />7 developed by the director. The director shall develop an <br />8 application form that consolidates the application under this <br />9 section and the application under section 286-303. The form <br />10 shall be furnished by the examiner of drivers and shall be <br />11 verified by the applicant before a person authorized to <br />1.2 administer oaths. The examiner of drivers and officers serving <br />13 under the examiner may administer the oaths without <br />14 charge. Each application for an instruction permit for a <br />15 category (1), (2), (3), or (4) license shall be accompanied by a <br />16 fee to be determined by the council of each county, and each <br />17 application for a provisional license or driver's license shall <br />OCS2018-0769/8/9/2018 4:07 PM <br />
