West Hawaii. Construction should begin in the fall 2019. <br />Helping low-income individuals and families avoid becoming homeless, especially extremely <br />low-income individuals and families and those who are: likely to become homeless after <br />being discharged from publicly funded institutions and systems of care (such as health care <br />facilities, mental health facilities, foster care and other youth facilities, and corrections <br />programs and institutions); and, receiving assistance from public or private agencies that <br />address housing, health, social services, employment, education, or youth needs <br />The County continues its partnerships with Hawaii First Federal Credit Union and Hawaiian Community <br />Assests, who provide free one-on-one financial education and counseling services, providing an <br />opportunity for individuals to address financial barriers. In May 2018, to provide for an increase in <br />services, the County applied for and was awarded with a planning grant to develop a plan to increase <br />capacity on delivery of financial education and counseling services. The County anticipates an award of <br />an implementation grant in late 2019, which will provide for an opportunity to increase services <br />throughout the island. <br />In 2018, the OHCD provided rental assistance through the Home Investment Partnerships Program <br />(HOME) funded, Tenant Based Rental Assistance (TBRA) Program. The TBRA program provided <br />preferences to the following groups of individuals: individuals experiencing homelessness, at risk of <br />becoming homeless, individuals living in transitional housing, survivors of domestic violence, individuals <br />aging out of foster care programs and family unification. <br />In addition, the OHCD received an award of 47 HUD VASH vouchers and 9 Mainstream Vouchers. <br />Helping homeless persons (especially chronically homeless individuals and families, families <br />with children, veterans and their families, and unaccompanied youth) make the transition to <br />permanent housing and independent living, including shortening the period of time that <br />individuals and families experience homelessness, facilitating access for homeless individuals <br />and families to affordable housing units, and preventing individuals and families who were <br />recently homeless from becoming homeless again <br />The County has fully implemented its Coordinated Entry System. The Coordinated Entry System (CES) <br />facilitates the coordination and management of resources and services through the crisis response <br />system. CES allows users to efficiently and effectively connect people to interventions that aim to <br />rapidly resolve their housing crisis. CES works to connect the highest need and most vulnerable persons <br />in the community to available housing and supportive services equitably. <br />The County convenes its homeless program partners at least monthly to address CES program concerns, <br />to coordinate resources and to discuss current and upcoming available housing. <br />CAPER 21 <br />OMB Control No::, 2506-0117 {exp.. 06/30/2018} <br />
