AGENCY: COUNTY OF HAWAI‘I - <br />Administrator’s Name: Kirby Shaw(State of Hawai‘i -Department of Health) <br /> Title: Disability and C <br /> <br />PHONE:(808) 974-TYPE:State <br />FAX: (808) 586-8129 <br />EMAIL: dcab@doh.hawaii.gov <br />WEBSITE: hawaii.gov/health/dcab/parking <br /> <br />MAIN OFFICE: <br /> <br /> <br />Honolulu, HI 96814 <br />ELIGIBILITY: You may qualify for a Disability Parking (DP) permit if a doctor/APRN who is licensed to practice <br />in Hawai‘i certifies that you have a mobility impairment. <br /> You are unable to walk two hundred feet without stopping to rest, due to a diagnosed arthritic, neurological, <br />orthopedic, renal, vascular, or oncological condition; <br /> You are unable walk without the use of, or assistance from, a brace, cane, crutch, another person, prosthetic <br />device, wheelchair, or other assistive device; <br /> You have restricted breathing due to lung disease to such an extent that your forced (respiratory) volume for one <br />second, (when measured by spirometry), is less than one liter, or the arterial oxygen tension is less than sixty <br />mm/hg on room air at rest; <br /> You use portable oxygen; or <br /> You have a cardiac condition to the extent that your functional limitations are classified in severity as Class III or <br />Class IV according to the standards set by the American Heart Association. <br /> <br />SUMMARY OF SERVICES: To establish a uniform parking system to enhance access and the safety of persons <br />with disabilities which limit or impair their ability to walk, and to conform to 23 CFR Part 1235, chapter 291, part <br />III, HRS. <br /> <br />SERVICES OFFERED: Issuance of Disability Parking (DP) permits. <br /> <br />ACCOMMODATIONS FOR THE DISABLED: Yes, building and office is ADA compliance. <br />INFORMATION/CERTIFICATES NEEDED WHEN APPLYING: Application Form Certified by Physician and <br />Applicant must present proof of identity. All forms of identification must be current or unexpired.
