Describe coordination with the Continuum of Care and efforts to address the needs of <br /> homeless persons (particularly chronically homeless individuals and families, families with <br /> children,veterans, and unaccompanied youth) and persons at risk of homelessness <br /> It takes collaboration among partners -advocates, community-based and faith-based service providers <br /> and government to address homelessness. Federal funding recognizes the importance of collaboration <br /> by requiring the Continuum of Care (CoC) organizations. In Hawaii,there are two - Partners in Care for <br /> Oahu and Bridging the Gap for the neighbor islands. Bridging the Gap is represented by Maul Homeless <br /> Alliance, Kauai Community Alliance and Community Alliance Partners(CAP) for Hawaii Island. For <br /> Hawaii Island,the following organizations are members of the CAP: Alano Club, Big Island Substance <br /> Abuse Council, Catholic Charities Hawaii, Child and Family Services, County of Hawaii, Going Home <br /> Hawaii, Habitat for Humanity, Hawaii County Economic Opportunity Council, Hawaii Island Horne for <br /> Recovery, Hawaii Island HIV/AIDs Foundation, HOPE Services Hawaii, Inter-faith Communities in Action, <br /> Na Kahua Hale o Ulu Wini - Hawai'i Affordable Properties, Neighborhood Place of Puna, Neighborhood <br /> Place of Kona, Project Vision Hawaii and Steadfast Housing. <br /> The CAP meets on the last Wednesday of each month both by video conference in Hilo and Kona. The <br /> CAP coordinates the annual application for federal and state funding and administers the annual Point in <br /> Time Count. <br /> Describe consultation with the Continuum(s) of Care that serves the jurisdiction's area in <br /> determining how to allocate ESG funds, develop performance standards and evaluate <br /> outcomes, and develop funding, policies and procedures for the administration of HMIS <br /> The County of Hawaii is not a recipient of Emergency Solutions Grant Funds. The State of Hawaii, <br /> Department of Human Services, Homeless Programs Office allocates these funds to eligible service <br /> programs through a competitive request for proposal process. <br /> Consolidated Plan HAWAII COUNTY 11 <br /> CMC Control No 2506-0117{exp.06/30/2018) <br />
