Is there a reason you don't put Rudy's last name? <br />APPELLANT SHOOK: I never asked for his last name at the time of working with him. <br />MR. MUKAL Why, does—he just had a name tag that said "Rudy" or something? <br />APPELLANT SHOOK: No, sir. <br />MR. MUKAL Okay. Now, on the last page the declaration it says, "I understand that <br />misrepresentation or the purposeful omission of facts called for on this form is reason to <br />disqualify me from further consideration and is grounds for termination if such items are a <br />busines necessity." Did you read that before you signed it on October 2, 2019? <br />APPELLANT SHOOK: Yes. <br />MR. MUKAL Now, the Hawaii Police Department, at the end of the form, there's a "Waiver of <br />Liability and Release Form"—it's two pages before Exhibit 2. <br />APPELLANT SHOOK: Before Exhibit 2 -two pages? <br />MR. MUKAL Yes. <br />REP. SHOOK: Excuse me, this was signed in front of a notary, yes? <br />MR. MUKAL I don't know. I wasn't there. I'm asking Mr. Shook whether or not he <br />remembers signing <br />REP. SHOOK: Just asking for clarification of the page. <br />MR.
