§ 5-4-1 H AWAI‘I C OUNTY C ODE <br />(5) If submitting plans, specifications, or construction documents pursuant to <br />section 464-13(b), Hawai‘i Revised Statutes, the applicant shall include proof <br />of recordation with the bureau of conveyances in accordance with section <br />464-13(c), Hawai‘i Revised Statutes; <br />(6) State the estimated value of the proposed work; <br />(7) Indicate that the owner or the owner’s authorized agent consents to the permit <br />application, by inclusion of the owner or the owner’s authorized agent’s <br />signature on the application; <br />(8) Declare the phases of work, if any, that will be performed by a contractor or <br />specialty contractor, or both, as required by law; <br />(9) Provide the name and license number of all specialty contractors involved in <br />the project, in compliance with the provisions of chapter 444, Hawai‘i Revised <br />Statutes; <br />(10) Be signed by the responsible managing employee or authorized employee of <br />each contractor designated in paragraphs (8) and (9); <br />(11) Provide email and telephone number of applicant or agent; <br />(12) Include a non-refundable permit plan review fee pursuant to section 5-7-1; and <br />(13) Contain any other information necessary to enforce the provisions of the <br />construction code as may be required by the authority having jurisdiction. <br />(b) Applications and required documents shall be filed in writing. After the authority <br />having jurisdiction implements its electronic filing system, applicants may submit <br />documents in writing or electronically. <br />(c) Applications that do not conform to the provisions of division 2 shall not be <br />accepted by the authority having jurisdiction. <br />(2020, ord 20-61, sec 2.) 5-4-1 <br />Section 5-4-2. Plans, specifications, and other data. <br />(a) The following documents shall be submitted with each application for a permit: <br />plans; specifications; engineering calculations; diagrams; soil investigation reports; <br />code search; special inspection and structural observation programs; and other data <br />as may be required by the authority having jurisdiction. <br />(b) Number of sets of documents to be submitted with each application: <br />(1) Two sets for single and two-family dwellings or town houses (R-3 Occupancy); <br />(2) Two sets for accessory structures; and <br />(3) Three sets for all other occupancies. <br />(c) Plans and specifications shall: <br />(1) Be drawn to scale upon substantial paper that is of sufficient size that the <br />plans and specifications when in print form, are legible without magnification; <br />(2) Be of sufficient clarity to indicate the nature and extent of work proposed and <br />show in detail that it will conform to the provisions of the construction code <br />and all applicable laws; <br />(3) Display the following information on the first sheet of each set of plans: <br />(A) The tax map key number of the job site; <br />SUPP. 9 (1-2021) <br />5-18 <br /> <br />
